I was rejected by a publication twice for a piece I only submitted once. Was their submission pipeline screwed up, or was there a lack of communication among the editors?

They said my article lacked insight and wasn’t up to their standards.

I accepted their notes and went on with my writing life. I didn’t submit a new article or resubmit the rejected piece; I let the publication be. A couple of weeks later, I received another rejection from them regarding my piece.

Was this deja vu or groundhog day for writers?
